How to submit a family for the holiday campaign:

Step One:

Prepare a short (2 min max) video describing your situation or the situation of the person you are submitting. You MUST start the video with your full name. Please have your video prepared before proceeding to step two. Videos are required for consideration.

Step Two:

After preparing your video, please fill out the form below to begin the process. You will upload your video after your form submission is completed. Please follow all steps carefully. Note: all fields are required for consideration.

Step Three:

After submission, please allow time for us to review all applicants. If selected, a member of our team will be in contact with you. Thank you.
